Should you let Volo operate on your eye in Baldur's Gate 3? We break down the choice, the Ersatz Eye reward, and whether the consequences can be undone.
Volo’s eye surgery is one of the most memorable early choices in Baldur’s Gate 3. During Act 1, after you rescue Volo from the Goblin Camp—specifically from the Shattered Sanctum, where he’s tied up and being “interviewed” by goblins—he joins your camp. Once he’s settled, he offers to remove the mind flayer tadpole from your brain. His method? An ice pick. Through your eye. It goes about as well as you’d expect.
He fails to remove the tadpole, but he replaces your eye with Volo’s Ersatz Eye, a permanent buff that grants See Invisibility within 9 meters. There’s no stat downside, no penalty to Perception, and no story punishment beyond the cosmetic change and the fact that you can’t get Auntie Ethel’s Hag Eye later. That last part is the source of most regret. The Hag’s Eye, obtained by accepting Auntie Ethel’s deal at her teahouse in the Putrid Bog, gives +1 to Intimidation but disadvantage on Perception. The two eyes are mutually exclusive.
This guide is for players who made the choice and want to fix it. The hard truth: Baldur’s Gate 3 has no in-game way to undo the surgery. You can’t visit Withers, use the Magic Mirror, or kill Volo to get your eye back. But depending on which mistake you made, you may still be able to fix it by reloading, talking to Volo again, or using a mod on PC.
If you want Volo’s Ersatz Eye and haven’t accepted yet:
If you refused Volo and want to change your mind:
If you accepted Volo’s surgery and regret it:
If you took Auntie Ethel’s Hag Eye and wish you had taken Volo’s:
Volo’s Ersatz Eye is generally the better choice. See Invisibility is useful throughout the game, especially in Act 2 and Act 3 when you fight invisible enemies like Balthazar’s minions, Yurgir’s merregons, and Orin’s cultists. It costs nothing, takes no equipment slot, and has no downside. The Hag’s Eye gives +1 Intimidation, which is nice for a face character who wants to bully NPCs, but disadvantage on Perception is a real penalty. Perception is one of the most common skill checks in the game, and it helps you spot traps, hidden doors, and ambushes. Unless you are building a dedicated intimidation character and don’t care about noticing things, Volo’s eye is the safer pick.
Save before every major dialogue with Volo or Auntie Ethel. On Honour Mode, you only have one save, so you cannot reload if you make a mistake. If you care about the choice, decide before you talk to either of them. If you are on PC and playing with mods, you can also use the “Appearance Edit Enhanced” mod or console commands to restore your eye, but that is not part of the base game.
The Magic Mirror in your camp can change your appearance, including eye color, but it does not remove the Ersatz Eye condition or restore your lost eye in cutscenes. Withers cannot help. Volo cannot reverse it. Killing Volo does nothing except remove a camp NPC.
Can I undo Volo’s eye surgery and get my eye back? No. There is no in-game way to reverse the surgery. You must reload a save made before you accepted Volo’s offer. On PC, you can use mods or console commands to remove the condition, but on console you are stuck with it.
Can I get both Volo’s Ersatz Eye and Auntie Ethel’s Hag Eye? No. They are mutually exclusive. If you accept Volo’s surgery, Auntie Ethel will not offer her eye deal. If you accept Auntie Ethel’s deal first, Volo will not perform his surgery. You have to choose one.
I refused Volo. Can I still get the eye? Yes, if you are still in Act 1 and Volo is in your camp. Talk to him again and accept the surgery. If he is gone, you have either advanced to Act 2 or missed the window. Reload a save before leaving Act 1.
